Lights, Camera, Surgery – The future of operating theatre technology

Every modern operating theatre is fitted out with state of the art technology to assist surgeons and their support-staff to perform surgeries with optimum efficiency. With an increasingly stretched healthcare system in Australia, many hospitals are implementing cameras into their operating theatres as another way to streamline the surgery process.

Transmitting and recording live video of surgeries or other medical procedures gives greater visibility to surgeons leading to increased confidence and precision and therefore improved patient safety. It can also prevent crowding within the O.R during procedures, with less staff needed in the room at any given time. Additionally, the technology can be used for tracking purposes as well as staff training and development.

While there are clearly many potential uses for this video technology in operating theatres, there are questions around where the technology is headed and where the best solution lies.

HD vs 4K

As the name implies, 4K refers to the horizontal resolution of 4000 pixels – four times the resolution of high definition. While an O.R would benefit from any video system, 4K technology offers a much clearer picture with a greater sense of depth. Surgeons are better able to distinguish small structures and perceive true colours which ultimately results in greater confidence during procedures.

Wireless vs Hard-wired

While there is still some uncertainty around wireless technology, more and more hospitals are adopting wireless solutions due to infrastructure and sustainability.

With respect to installation, wireless involves a much simpler installation process. Hardwired camera and video systems can be costly to install, inflexible for operation and can result in more maintenance over the long-term. Wireless systems can also be easily relocated from room to room during inevitable renovations or expansions.

Latency

In terms of video transmission, latency is a value to look out for when selecting an AV management system to integrate with your camera system. Latency is the total delay between when a frame is captured and the exact instant it is displayed on the receiving device. Latency is typically expressed in terms of time, most commonly in milliseconds or seconds. A low latency value is best, particularly within the operating theatre where accuracy is critical.

Focus

Like any camera, focus and iris are important functions of any O.R camera system. Iris controls exposure while focus controls the lens to form a clear and sharply defined image on the focal pane.

Most cameras come with both an automatic and manual control, however some don’t. Having the option of manual focus gives surgeons flexibility with the images being transmitted to the monitor. With auto focus there is also a risk of the infrared assist beam interfering with robotics, which for the most part is not an issue outside of robotic surgeries. However, choosing a camera with both auto and manual focus options is a good way to future-proof your video system.

Integration

Within the O.R, integration refers to functionally connecting theatre lights, audio-visual equipment and other equipment to a central console. An integrated O.R over a non-integrated O.R means the surgeon or circulating nurse no longer needs to navigate the room during surgery; able to access devices from a central control station.

As mentioned earlier, an integration system with low latency is best to ensure your images are being transmitted promptly. It’s also important to source a system that is scalable and vendor neutral for future-proofing.

4K camera systems are set to be the next standard technology adoption in operating theatres in Australia. With more and more options available on the market, it’s important to do your research and find a solution that not only meets the needs of your O.R today, but into the future.

Top 7 factors to consider when selecting a washer disinfector

It wasn’t that long ago that most facilities cleaned their instruments by hand before sterilisation. However, these days in order to comply with Australian standards, in particular AS 4187; a facility needs to ensure efficient preparation of medical instruments. Where the accuracy of hand-washing was dependent on the individual personnel’s assessment, automated washing now leaves very little room for error during re-processing. Still, not all washer disinfectors are created equal and there are many variables to consider when selecting a new washer disinfector for your facility.

Effective drying capability

Perhaps the most important factor to consider when selecting a washer disinfector for your facility is the units drying capability. While most washer disinfectors are equipped with a heated drying cycle and a forced air drying system, selecting a unit with additional drying capabilities results in a more thorough drying process.

Finding a unit with customisable drying cycles is a valuable feature for a washer disinfector. Being able to modify cycle times and parameters means being able to meet the individual requirements of your facility, your load and the RMD’s that you are processing.

Additionally, if you can source washer disinfectors with in-chamber fans this can also greatly accelerate the drying process, with air-flow able to reach all rack levels evenly.

Dosing pumps

Confirm your washer disinfector has a minimum of two dosing pumps. This ensures the chemical agent is dispensed quickly to reduce the cycle time. Additionally, this feature gives the facility the freedom to purchase any brand of chemicals they prefer dependent on their budget and current supplier contracts.

Future proofing

Purchasing a new washer disinfector is a huge investment, so it’s critical to future proof by sourcing a unit that not only meets the needs of your facility now, but in the years to come. It’s important to consider the following:

Electronic traceability – AS 4187 states that HSO’s should be working towards an electronic tracking/process record system. Ensuring your washer disinfector has electronic capability to tray or instrument level is extremely imperative as facilities move towards completely paperless systems.

Robotics – With Australia poised for a boom in robotic surgery over the next few years, selecting a unit that is compatible with robotic instrument preparation (in particular the Da Vinci Surgical System) would be a strategic move for larger hospitals.

Remote maintenance

Another important factor to consider is the serviceability of your washer disinfector. Access to remote maintenance can greatly reduce time and service costs, with a facility’s service provider able to identify errors and diagnose problems from off-site.

Automatic loading and unloading

For larger CSSD’s the opportunity to reduce handling can save a considerable amount of time. Finding a washer disinfector that has automatic loading and unloading of carts results in less reliance on staff and significantly lower operational costs.

Ergonomics

While this seems obvious to consider, quality construction can significantly improve the usability of your washer disinfector.

For example, quiet operation can signal a well-constructed door and chamber. Sound-proof and insulated doors can greatly reduce noise in the CSSD and make for a more comfortable workspace. Some washer disinfectors also use inflatable door seals which properly seal the chamber during cycles, eliminating any residual noise.

Another important question to be asked in regards to ergonomics is whether the unit offers easy access to the electronics and chemical drawer for serviceability. Access through the front of the unit means washers can be placed side-by-side within the CSSD for more efficient use of space, and your service provider or biomedical team can perform maintenance easily.

In regards to the chamber and racks, make note of your washer’s capacity and the versatility of your racks. Some modern units use identical rack connections on all sides, meaning racks can be inserted from any side and therefore eliminating human error.

Even small differences in features can also make a huge difference to everyday operations. For example, some washers use coloured lighting to indicate cycle phases or large fonts are used to display cycle information on the user interface. Both of these features allow CSSD staff to easily identify cycles and monitor work-flow from a distance.

Economical options

Most modern washer disinfectors come with the option of a pre-heat tank and recovery tank.

A pre-heat tank means water can be pre-heated before entering the chamber, saving anywhere from 10 to 20 minutes where the water would typically heat once in the chamber. Having this option allows more cycles to be performed over the day for increased efficiency.

A recovery tank offers an intermediate storage of process water and recycles the rinse water. This ensures a faster cycle time and reduces water and energy consumption.

AS/NZS 4187:2014 and Reverse Osmosis

With the release of the AS/NZS 4187:2014 standard, Australian day surgeries and hospitals will be required to comply with a range of new requirements for processing reusable medical devices (RMD’s) by December 2021. A particular point of interest is Section 7.2.3 and Table 7.2 – Water Quality Used for Processing RMDs. This section states that water supplied to the reprocessing facility, such as a Central Sterile Supply Department, shall be of a suitable quality for its intended purpose1.

Essentially, to achieve the water quality specified in Table 7.2 the local water supply needs to be treated through a water purification system. Reverse osmosis filters are extremely efficient and effective for the removal of microorganisms and most residual chemicals from water2. In addition to variables such as cost effectiveness, ease of installation, functionality, and footprint, reverse osmosis is by far the most ideal water purification method.

However, what factors does a health facility need to consider before making a decision on the best way to comply with these requirements?

Pipeline and Ring Main

First and foremost it is important for each facility to consider if they’re current pipeline is compatible with a reverse osmosis (R.O) system. Generally, metal piping should be avoided for use with high purity water3 as R.O water is typically aggressive, and the zinc component in brass is easily lost to the water4.

While there are benefits to a stainless steel ring main such as its longevity; installation and material cost is high. Furthermore, stainless steel has a rough surface finish with crevices that can result in bacterial attachment and early biofilm formation5. A suitable ring main alternative to both brass and stainless steel is un-plasticized polyvinyl chloride (UPVC). Characteristically UPVC has an ultra-smooth internal finish with glued joints which is smoother than traditional stainless steel. Additionally, UPVC is substantially less expensive compared to metal in regards to both manufacture and installation.

Image 1 - Bacteria in cracks or brush marks on a stainless steel surface following a routine sanitisation5

Storage and pumps

Another important factor to consider is suitable storage of the R.O water to maintain water quality. A pressure tank is a commonly used water storage system which employs a water bladder to control the tank water level. The biggest weakness with this type of system is that ultimately the water sits inside the tank with no circulation. A certain amount of air pressure is also needed to ensure the pump does not start to short cycle.

Ideally, a storage tank integrated with recirculation pumps would prevent a biofilm from developing. Some new tanks can also be equipped with a high and low level water sensor and a secondary over-flow prevention device. These features can prevent the pump from running dry or tank overflow in the event of a sensor failure.

Water Hardness

Dependent on your facilities location, you may also require a water softener which will remove minerals such as calcium, magnesium and iron ions from your local water before the R.O process. Hard water can cause scale during the filtration process, making it extremely difficult for the R.O’s membrane to function and as a result it lowers the quality of the water6. Your local water provider will be able to supply you with information on your local water supply, or alternatively your R.O system supplier will carry out research that will reveal if you require a water softener.

Below is some general information on water hardness in Australia:

The typical hardness level of water delivered in Melbourne’s South East area is between 12 – 45 mg/L. This means that the water being supplied to South East Water customers is considered "soft" by international standards7

Greater Sydney area water is considered ‘slightly hard’9

The water hardness in Perth and the rest of Western Australia is considered ‘hard’, particularly in country areas10

South East Queensland water is considered ‘hard’ to ‘moderately hard’8

Filters and Disinfection

One of the most important features of a quality R.O system is an endotoxin filter. Most new R.O systems are designed with an endotoxin filter which filters out bacterial endotoxins. In addition to an endotoxin filter, an ultraviolet (UV) disinfection system will also prevent a microbial population from developing. When UV light enters a microorganism the energy damages the microorganism’s cellular function rendering it inactive and averting reproduction11.

Image 2 – U.V light inactivation of microorganisms

Traditional chemical disinfection methods require multiple flush cycles to remove residual chemicals. By combining U.V disinfection and an ultra-smooth pipeline, ring main sanitisations are only required every 12 months (carried out by your service provider). The end result being a simplified Reverse Osmosis system with fewer points of failure and lower operating and service costs.

In summary, there is little doubt that an R.O system is the most efficient and cost effective method of meeting the water requirements of AS/NZS 4187:2014. The key is finding the most practical innovations in R.O available at this time.

“Its important facilities take advantage of the new reverse osmosis technology that is now available” Kurt Hogan explains.

The National Product Manager of Mediquip in Brisbane says that the developments in U.V disinfection and research on UPVC as a suitable alternative to metal ring mains, means that facilities have better options than ever before.

“In the last two years Mediquip have had to bust a few long-standing myths on R.O. We’ve done the research and we simply encourage facilities to do the same.”

Mediquip delivers a holistic CSSD solution to Renaissant Aesthetic Health in East Brisbane

Renaissant Aesthetic Health opened in East Brisbane in late August this year after a whirlwind construction over eight months. Mediquip, a leading Australian biomedical sales and service provider were at the forefront of the project, delivering an array of state-of-the-art equipment including two fully integrated theatres and a holistic CSSD solution to the day hospital.

With a number of Mediquip CSSD upgrades over the last twelve months receiving accreditation to AS/NZS 4187: 2014, Renaissant Aesthetic Health knew they were in safe hands.

“When choosing someone to partner with in our new Day Hospital there are many things to be considered, and Mediquip met all of our requirements,” explains Renaissant Aesthetic Health CEO Rachael Cook.

“Their knowledge of our industry and the requirements needed for us to provide the best facility possible to our patients was second to none. They have been integral in helping us be one of the first facilities to meet the new AS/NZS 4187:2014 standards.”

With the new version of AS/NZS 4187 becoming an impending hurdle for hospitals and day hospitals, Mediquip was surprised to find a gap in the market with limited options for customers seeking a complete CSSD solution.

Kyle Cederblad, Product Specialist at Mediquip, said that many companies fall short, offering only CSSD equipment or consumables — not both.

“Whether it’s a new facility or a renovation our customers need washers, sterilisers, reverse osmosis systems that help them comply with the new standard, and they need to be able to monitor this equipment correctly. It can get complicated when staff is using equipment and consumables that don’t fit well with each other, or don’t have suitable technical support,” Mr Cederblad said.

Installing a Uniflow Reverse Osmosis System along with a DGM Washer Disinfector and Steriliser, Mediquip are certain that Renaissant Aesthetic Health can deliver quality and compliant sterilisation and reprocessing while also meeting AS/NZS 4187.

“We understand the challenges associated with compliance within the Central Sterile Supply Department. Therefore we do a lot of research and make sure our products meet specific requirements.”

Mr Cederblad explains those requirements include ease-of-use to streamline facility processes, innovative technology to keep up with industry developments, and most of all quality products that will last.

“For example, we sourced our Uniflow Reverse Osmosis System specifically with our customer in mind. Our number one consideration was how best to help our customers reach accreditation to the new standard.”

Mediquip’s consumables are no exception to quality either, recently introducing the TRAQS Traceability System including routine monitoring consumables and washer process chemicals. The company has also introduced a range of Famos equipment from heat sealers to roll holders.

“We loved how Mediquip have a solution for all of our needs,” said Ms Cook.

“Having experts we can rely on for not only the capital equipment, but the products, consumables and tracking system was of great interest to us.”

Mediquip can now add Renaissant Aesthetic Health to a growing list of successful projects the company has spearheaded in Australia this year. Thirty-eight Simeon Theatre lights were installed at Gold Coast University Hospital from June to September, in addition to another complete CSSD solution installed at the new ultra-modern Westside Private Facility in Indooroopilly.

“We already have a number of CSSD fit-outs scheduled over the next few months and only expect more and more in the lead up to the December 2021 deadline.”

Mr Cederblad continues, “And with the feedback we get from customers like Rachael from Renaissant, it makes it each successful project even more worthwhile.”

Renaissant Aesthetic Health is located at 45 Wellington Road, East Brisbane.

ABA100 - Leading Business by Innovation

Mediquip has been recognised as an ABA100 Winner in The Australian Business Awards 2018 for Service Excellence. Now in their thirteenth year, The Australian Business Awards program provides notable opportunities for high-performing organisations which implement world-class business initiatives and develop innovative products and services to be acknowledged and honoured for their achievements both nationally and internationally. The national winners are benchmarked in the international chapter of the program at The World Business Awards whereby participants are provided with the unique opportunity to benchmark themselves against the top performers globally.

Chris Daley, National Service Manager of Mediquip says the award affirms their commitment to continuous improvement and innovative business processes. “Mediquip are extremely grateful to receive the ABA100 Award for Service Excellence this year. A longstanding philosophy within Mediquip is that if we stay dedicated to our customers, they will continue to return. We strive to ensure complete satisfaction with our products and service capabilities, and our Medical Asset Administration System is no exception. This award recognises Mediquip’s unwavering dedication to our over 3000 service customers across Australia with the support of our innovative asset management tool M.A.A.S.”

First developed in 2003, the M.A.A.S database is unique to the Australian medical service industry. M.A.A.S was designed by Mediquip to eliminate the need for customers to manage their own assets. All equipment records are managed by Mediquip through an intranet/extranet system. Each customer site receives an individual login and is able to access their records at any time. Mediquip has over 3000 regular service customers managed through M.A.A.S from major hospital groups with thousands of assets, to local medical centres, dental practices and vet clinics with one asset.

“With technology rapidly evolving, it is essential that organisations remain ahead of the curve by continuously exploring and embracing new technologies and investing in the education, training and development of their workforce in order remain competitive,” Australian Business Awards, Program Director, Tara Johnston said.

“High performing organisations that have adopted an innovation culture to achieve optimal levels of productivity and performance have been recognised in this year’s ABA100,” Ms Johnston added.

Each year the ABA100 Winners are recognised for their commitment to business and product innovation and for their achievements in transforming business practices and end user experiences. Organisational participation includes private companies, publicly listed companies, multi-national subsidiaries, non-government organisations, educational institutions, franchise systems, partnerships, government departments, government agencies, local government and statutory bodies operating in Australia.

The World Business Awards Framework (WBA Framework) is utilised as a structured model of assessment that enables the participating organisations to be benchmarked against world class performance standards. The Framework consists of specialised assessment modules pertaining to the evaluation criteria for each of the award categories. The criteria and sub-criteria provide a robust set of requirements that are used as the methodology for benchmarking and learning among the participating organisations.

Organisations that demonstrate the core values of business innovation, product innovation, technological achievement and employee engagement are recognised via a set of established business and product award categories. The process culminates in the selection of organisations which implement world-class business initiatives, projects, products and services. The award recipients are recognised as the elite of their industry earning a place amongst a distinguished group by having their achievements acknowledged and celebrated.

Life-threatening and complicated childbirths can now be trained for on a life-like human simulator, only the second of its kind in New Zealand, at Waikato Hospital.

The mannequin or wireless birth simulator, known as Lucy Sim, is so realistic she has a heart rate, blood pressure and even makes sounds.

Doctors, nurses and midwives practised on the mannequin on Monday in a training exercise to get to grips with the hospital's newly refurbished delivery suite theatre.

Waikato Hospital consultant anaesthetist and anaesthetic simulation faculty organiser Dr Jeff Hoskins said the test was to make sure the upgraded operating room was safe for patient procedures.

The training session included two assisted deliveries using forceps and ventouse, as well as an emergency caesarean, all relatively common obstetric possibilities but which carry risk.

"We're using a highly advanced mannequin, a robot essentially, which is able to replicate the normal processes of delivery and also is able to suffer some of the crises that are common during the delivery of a baby."

The mannequin, made by CAE Healthcare at a cost of $145,000, can be given anaesthetic and can also be resuscitated.

"We're using this robot so that we can get as robust a test as possible of all of our crisis response before we put patients back into that environment."

Hoskins said the simulator feels exactly like dealing with a real patient.

"Often when we train with simulation people can't believe how realistic it is and it usually brings a lot of issues to the surface which we can debrief and teach better crisis response."

Hoskins said Waikato District Health Board was fortunate to have the robot, one of the most advanced mannequin simulators in the world, with Waitemata DHB the only other health board in New Zealand to have one.

"There's nothing like putting people in their real teams in their real clinical environment and having something very rare happen so that they can train to deal with it when it happens in a patient."

Waikato DHB charge midwife manager for delivery suite Celeste Pon said the simulator provides an amazing opportunity for training.

"This was really about testing the absolute worst-case scenarios. Research has shown that when we are working together as a team it's really important to be able to get together in simulation."

The training involved anaesthetists, theatre nurses, midwives and an obstetrician.

Midwife educator Deborah Cox said midwifery, nursing and multidisciplinary emergency training days have all been revamped since Lucy's arrival.

"In the past we have had to rely on willing participants to role play, and obviously there were severe limitations on what we could show or do," Cox said.

"With Lucy, it allows a much more realistic scenario and a lot more realistic detail for staff to practice with.

"She's a really important member of our maternity healthcare team."

The DHB's simulation centre director Rob Sinclair said the robot was an excellent addition to Waikato Hospital's suite of human patient simulators which included a premature baby simulator, and was purchased after much research of the options.

He said midwives had embraced simulation-based education and the realism and practice that can be achieved in a safe environment to acquire new skills and communication behaviours - without risking real patients.

We are now three quarters of the way through 2017 and with many more projects in the pipeline, our team are gearing up for the remainder of the year.

Firstly, our warehouse team have been extremely busy of late with in-coming stock, and out-going customer orders. Over 30 pallets of Pinkies were sent out to QLD Health back in July! Plus, we had plenty of O-Two circuits, Stainless Steel Trolleys and Wallcur Practi Products arrive!

The same month Bronwyn jetted off to Queenstown in New Zealand for CTVP, an ANZCA ultrasound event, with our Vimedix Ultrasound Simulator. Our installation team was busy with a CT40 install at Mt Gravatt Eye Tech Day Surgery, 60L Tiche Steriliser at QSCAN Annerley, a DGM Washer at Reservoir Private Hospital, and more Simeon Businessline lights at Windsor Private Hospital.

August was a big month for conferences, with a major event for each of our departments. Our Healthcare team attended the Day Hospitals Australia annual event in Sydney, while our Simulation team were also in Sydney for Sim Congress 2017, and were able to giveaway a Tellyes Simulator to a lucky delegate. Closer to our Head Office, our Veterinary Specialists hit up the Gold Coast for FASAVA which combined four veterinary conferences into one. Mediquip has more conferences on the horizon with ASUM in Melbourne and AMMA in Brisbane in October, as well as the annual ADSNA event in Adelaide, and the UQ Vet Nurses Conference in Gatton. Be sure to check our website to find out where we’ll see you next!

You will also notice some changes to our online platform over the next few weeks as we launch our new website.

It has been another few busy months here at Mediquip and it doesn’t seem to be slowing down any time soon. We have had an abundance of installations, workshops and conferences keeping our team busy throughout Australia.

In May, Bronwyn travelled to Perth for Demos and Respiratory Workshops with our Vimedix Ultrasound Simulator, while Dana visited Dubbo for training with the Royal Flying Doctor Service who recently purchased our Lucina and Apollo simulators. Later in the month, Dana was off to Adelaide for the annual Spark of Life event hosted by the Australian Resuscitation Council and took along our CPR Training models.

Even more exciting news was the team at the Werribee Open Range Zoo in Victoria using our Bionet BM3 monitor for a health-check procedure on Yankini the silver-back gorilla. You can check out some photos and a video of the procedure on our Facebook and Twitter pages or HERE.

A DGM AND 150 was installed at Albany Creek Veterinary Surgery and Simeon Business Line Theatre Lights were installed at Veterinary Specialist Services in Underwood, both in Brisbane. Kyle, Ben and Paul carried out a double installation of Simeon Highline Theatre lights at Sunnybank Private Hospital, and just recently Kyle installed a DGM AND 100 steriliser at HCF in Hornsby NSW along with Kurt.

Our calendar has also been filled with events with the Annual Australian Sonographers Association Conference which Bronwyn and Leilani attended with CAE Healthcare in Brisbane. The VimedixAR was a hit with delegates keen to have a try using the hololens which allows users to scan in an augmented reality. The same weekend Cam and Lisa were in Melbourne for the Annual Australian Veterinary Association Conference. We were pleased to announce 2 winners of our conference competition – Alan Forte from Racecourse Road Veterinary Clinic and Rowan Smith from Warwick Town and Country Vet. They each won wine, chocolates and a $300 service voucher!

January and February have been very productive for our team and our clients, and March seems to be no different.

In January we released our new Vet Product Catalogue for the year which includes some exciting new products such as the Vitaflex CR System and the portable V3Vet Colour Doppler Ultrasound System. You can have a look at the full catalogue on our website.

Last month we launched the SunTech CT40, which will also be included in our upcoming Healthcare Product Catalogue. Our team are super excited about this spot-check device as it includes an averaging protocol for more accurate results, and it’s modular design means that clients can attach modules for extra parameters if needed.

Also in February our qualified technicians completed a light installation at the Windsor Private Hospital in Melbourne. The installation included Simeon LED lights with SimCAM, SimSTREAM and a Wall Control Panel. Simeon, a German based company, has applied the latest LED and reflector technology to their products and, as a result, produced superior theatre and examination lights. We think Windsor Private Hospital will be very pleased with the new system.

A DGM washer disinfector was also installed at Windsor. The installation was successful, and even managed to make it into DGM’s international newsletter, and our team were delighted by the shout-out.

Mediquip also has many exciting projects on the horizon in the coming months. Keep an eye out for our new Healthcare Product Catalogue which will be available soon. Our very own Veterinary Sales Consultant Cameron Jansen will get to sit in on a gorilla procedure at the Werribee Open Range Zoo later this month with one of our Bionet patient monitors used during the procedure.

Cameron will then be jetting off to Sydney to meet up with Lisa Prince, our other Veterinary Sales Consultant, to attend the Merial CE Event on the 25th & 26th of March. Mediquip are supporting a Ventilation Workshop over that weekend, with Dr Rob Webster, Dr Ellie Leister and Dr Lisa Chimes. Many of you may know Dr Lisa Chimes from Bondi Vet.

If you would like to keep up to date with Mediquip follow us on social media
